A single independent assessment model will provide a clear picture of people’s functional needs, goals and opportunities for independence.
Those who need ongoing services will agree a plan with the assessor that includes a budget that aligns with their needs, and a suite of services that can be met from within the budget.
A referral to services will be provided within a month of assessment, as the program will be properly funded to meet growth in the population, increasing frailty, and people’s preference for support in their homes.
Older people will be able to access services from one or multiple providers. The choice of providers will be much greater than it is today, with no more grant-funded monopolies.
People will be able to change their services as needs arise (from within their budget) from an approved a service list, with limits on cleaning and gardening.
Services for people with diverse needs or living in remote communities will be assured by giving extra funding to those providers where needed. Consideration of grants to meet the needs of people in areas with low client numbers.
When people need to be re-assessed for increased needs they will be seen by the same organisation that did the original assessment.
